Just like any new business, the owner/developer of a food facility must first obtain the appropriate zoning, building, electrical and plumbing permits.  For more information, click on the tabs to the left of this page, or phone the City Building/Zoning Office at 419-468-2642.

 In addition, plans must be submitted to the City Health Department for review prior to construction.  Plans and specifications for a proposed food facility shall be legible, be drawn reasonably to scale, and shall include the following:

bullet

Name and address of operation;

bullet

Name, address and phone number of owner;

bullet

The type of operation or establishment;

bullet

The total area to be for the food service operation or retail food establishment including square footage, and plans for all portions of the premises in which the food service operation or retail food establishment is to be conducted;

bullet

Entrances and exits;

bullet

Building materials and surface finishes to be used;

bullet

Plan of lighting, both natural and artificial, with foot-candles indicated for critical surfaces;

bullet

Location, number and types of plumbing fixtures, including all water supply facilities;

bullet

A floor plan showing the general layout of equipment;

bullet

An equipment list with manufacturer and model numbers (cut sheets are acceptable). Only commercially designed equipment that is approved by an acceptable testing agency is to be installed;

bullet

A menu or list of foods to be sold;

The Health Department has thirty (30) days after receiving food facility plans to review and respond to said plans.  The plan review fee is 30% of the license fee, which will be determined by the classification of the facility, and is payable at the time plans are submitted.  Once plans are approved, construction and development must be according to the approved plans, unless a plan revision is filed in writing.  Please contact the Health Department at least ten (10) days in advance of the anticipated opening date for a pre-licensing inspection.  Application may be made at that time for a food facility license.  For current fees see 2009 Environmental Health Fee Schedule.
