Vital StatisticsCertified copies of birth and death certificates for those who were born, or have died, within Galion City Limits may be obtained from the Galion City Health Department. Birth or death records from December, 1908 to the present time are on file. All certified copies are $25.00 each effective October 16, 2009. Uncertified (plain paper) copies of birth or death certificates are not issued. “For your convenience, you can process online requests through an independent company that Galion City Health Department has partnered with to provide you this service; VitalChek Network, Inc. VitalChek can be reached through their website; www.VitalChek.com. An additional fee is charged by VitalChek for this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.” To place an order by e-mail, go to www.VitalChek.com and follow the instructions given. Credit or debit card orders are subject to a $7.00 surcharge for credit card and debit card approval via the Vital Chek system. This is in addition to the $25.00 fee for the certified copies. Information required to obtain a birth record is the person’s name, birth date, mother’s maiden name and father’s name. To obtain a death record, the person’s name and date of death are required. To request a certificate by mail simply print an application form from this site, fill it out and send a money order ($25.00 each) for the document(s) ordered. We don’t accept personal checks by mail. Please enclose a self-addressed, stamped envelope.
